Updated AAP Guidance on Managing Atopic Dermatitis in Children

A new clinical report published in Pediatrics provides revised guidance from the American Academy of Pediatrics (AAP) on treating children with atopic dermatitis (AD). The report notes that many pediatricians may not feel comfortable managing AD and offers a clear, practical approach. Successful treatment involves three main steps: maintaining skin care, topical anti-inflammatory therapy, and avoiding triggers.

It is recommended that children bathe daily or every other day using lukewarm water and a gentle cleanser. Products containing fragrance, dyes, or essential oils can cause skin irritation or allergic reactions and should be avoided. Topical corticosteroids or calcineurin inhibitors should be applied once or twice daily to active eczema patches and not mixed with topical emollients unless specifically instructed to avoid reducing the efficacy of the medication. Moisturizers should be applied generously over the whole body at least once a day, especially after bathing. Flares can be reduced by avoiding triggers and using topical anti-inflammatory medicines proactively. Dupilumab and Janus kinase inhibitors are emerging treatment options for AD.

The report emphasizes that severe or complex cases should be referred to subspecialists, and collaboration between primary care providers and specialists can help optimize care.
