Motivational Interviewing: Putting Compassion at the Center of Obesity Care

Motivational interviewing combines evidence-based communication with empathy and autonomy support, making it a valuable tool for sustainable obesity management across all age groups.

Obesity is a complex chronic condition shaped by biological, psychological, and social determinants, requiring integrated and long-term management strategies. Traditional approaches based primarily on dietary advice and physical activity recommendations are often insufficient to produce sustained behavior change, particularly when delivered in a directive or information-heavy manner. Emerging evidence highlights the importance of communication quality in determining the success of obesity interventions.

A narrative review of motivational interviewing (MI) underscores its value as a patient-centered communication approach that enhances intrinsic motivation and supports sustainable lifestyle modification. Rooted in empathy, collaboration, acceptance, and respect for autonomy, MI helps individuals resolve ambivalence toward behavior change while strengthening personal commitment to health goals. Rather than instructing patients, clinicians using MI engage in structured, supportive dialogue that elicits the individual’s own reasons for change.


The evidence base for MI is extensive, with more than 1,900 controlled trials and over 200 systematic reviews. In obesity management, findings from randomized controlled trials and meta-analyses indicate modest but meaningful improvements in weight outcomes, with some studies reporting clinically relevant weight loss and increased likelihood of achieving ≥5% body weight reduction. MI has also been associated with improved self-efficacy, healthier dietary behaviors, increased physical activity, and better psychosocial outcomes in both adults and pediatric populations, particularly when embedded in multidisciplinary and family-based programs.

Importantly, the review highlights the role of MI in addressing weight stigma and reducing the negative psychological burden associated with obesity. By fostering non-judgmental, respectful communication, MI creates a therapeutic environment that supports engagement, autonomy, and long-term adherence. These findings reinforce MI as a valuable adjunct in modern obesity care, where empathy and communication are as critical as clinical intervention.
