Atypical Right Atrial Mass in a Neonate: Myxoid Spindle Cell Tumor Case Report

Introduction

Cardiac tumors containing spindle cells are typically associated with malignant cardiac sarcomas. Reports of benign spindle cell tumors in the neonatal period are exceedingly rare. We present the case of a neonate with progressive respiratory distress and a patent ductus arteriosus (PDA), ultimately diagnosed with a benign right atrial myxoid spindle cell tumor.

Case Presentation

A neonate was referred with symptoms of respiratory distress and suspected sepsis. Initial echocardiography revealed a small atrial septal defect, PDA, and a heterogeneous, rounded mass within the right atrium. Given the clinical instability, further evaluation was pursued.

Surgical excision of the mass was performed. Histopathological analysis confirmed the diagnosis of a right atrial myxoid spindle cell tumor with no evidence of local invasion, indicating a benign lesion. The postoperative course was uneventful, and follow-up echocardiography demonstrated normal cardiac structure and function.

Conclusion

This case highlights the importance of broad differential diagnosis when evaluating infants with signs suggestive of congenital heart disease. Neoplastic processes, though rare, must be considered. Importantly, the presence of spindle cells in cardiac pathology does not invariably predict malignancy. Benign spindle cell tumors can occur in neonates and may have excellent outcomes following timely surgical resection.
