Obesity and Osteoarthritis: The Connection Goes Beyond Body Weight

The link between obesity and osteoarthritis extends beyond the added pressure on joints, with weight management offering a practical way to support joint health and mobility.

Osteoarthritis can make everyday activities such as walking, climbing stairs and getting up from a chair increasingly difficult. While ageing plays a role in its development, excess body weight is another important factor, particularly in knee osteoarthritis. The strain on the joints is only part of the picture; the inflammatory effects of excess fat may also contribute to joint damage.

Evidence points to a higher risk of osteoarthritis as body mass index (BMI) increases. One population-based study found that people with grade II obesity had up to 4.7 times the risk of knee osteoarthritis compared with those whose BMI was within the normal range. Although this association does not establish cause and effect, it highlights the importance of addressing excess weight in joint health.

The effects of obesity are not limited to the additional load carried by the knees. Fat tissue releases inflammatory substances, including leptin, tumour necrosis factor-alpha and interleukin-6, which may contribute to cartilage breakdown. Pain and reduced mobility can, in turn, make physical activity more challenging, adding to the burden of the condition.

Weight management deserves a place in the broader approach to osteoarthritis care. Studies suggest that losing 5–10% of body weight may help ease pain and improve physical function and quality of life in people with obesity and knee osteoarthritis. A balanced diet and suitable physical activity can support gradual, sustainable weight loss.

Addressing obesity alongside joint symptoms may help people move more comfortably and maintain independence in daily life.
