A Rare Case of Second-Trimester Ovarian Pregnancy Rupture With Neurologic Presentation

Ovarian pregnancies account for approximately 3% of all ectopic pregnancies. Second-trimester ovarian pregnancies are extremely rare. We report a case of ruptured ovarian pregnancy in the second trimester presenting with abrupt mental status change. A 26-year-old woman with no antenatal care arrived at the emergency department with coma and respiratory failure. Pelvic ultrasound revealed massive pelvic fluid collection. Exploratory laparotomy identified a ruptured right ovarian pregnancy with the fetus expelled into the pelvic cavity. This case highlights the importance of antenatal care and timely ultrasonographic evaluation to confirm intrauterine pregnancy.

A 26-year-old woman with no antenatal care presented in a comatose state with severe hypoxia and hemodynamic instability. Initial cardiopulmonary evaluation was unremarkable, but rapidly developing abdominal distension raised concern for internal bleeding. Pelvic ultrasound showed significant pelvic fluid, and hemoglobin was 4.8 g/dL.

Emergency laparotomy revealed massive hemoperitoneum and a ruptured right ovarian pregnancy with the fetus expelled into the pelvic cavity. Right oophorectomy was performed, complicated by disseminated intravascular coagulation requiring extensive transfusion. The patient also developed transient acute kidney injury but recovered with supportive care.

Histopathology confirmed ovarian pregnancy. This rare second-trimester presentation highlights the critical need for timely antenatal care and routine ultrasound to confirm intrauterine implantation.
